We will be offering free physicals on Monday, June 12th for any student in grades 6th-11th who plans to play a sport next school year. These will be offered in order of who signs up first. Please SIGN UP HERE no later than Friday, June 9th.
You will be required to upload an Interval Health History form to the sign up sheet. On the form where it says "Grade", please choose the grade your child will be going into in the fall. The Interval Health History form must be filled out and submitted by June 9th or your child will not be able to receive a physical even if they are signed up.
